Shiloh Family Community Services  primary goal is to preserve the individuals independence and maintenance of healthy daily living through providing community linkages, role definition, and communication skills training that will equip the individuals with the knowledge, skills and abilities to recognize personal danger, establish or maintain normal interpersonal relationships, recognize significantly inappropriate social behavior  assist with basic independent living skills to prevent hospitalization, homelessness or isolation from social support as a result of conflicts with family or community.  Under the supervision of the Clinical Director, Program Director, QMHP's will provide: case management, direct skills building/training as it pertains to enhancing ADL Skills and supportive activities to help the Individual manage their mental illness in the community and the workplace as well as assistance with medication management and monitoring concerns with overall health, nutrition and physical condition.  QMHP will work directly on identified goals which address issues at home and in the community.  All services will be in accordance with agency policies and procedures.  Individuals/families may utilize any and all program services available, as appropriate.
